Submitting a registration form online
The registration form (utilising Google forms) will require some information to be gathered before the form may be finalised.
The information required are:

Names of 3 participants assigned to the math quiz

Names of 3 participants assigned to the science quiz
-
NRIC number of all 6 participants

The grade of each participant (primary 5 or 6)

Dietary requirements of participants (e.g. vegetarian)

Medical condition or allergies of participants
Once all the information has been obtained, it should have been keyed into a registration form. [Note: registration has closed.]
Registration fee
Please note that a registration fee of S$15 will be applicable to each participant that is entered into the challenge. This fee must be paid through GIRO or IFAAS.
Payment details through GIRO or IFAAS can be found on the invitation letter issued to your school.
Deadlines for registration and payment
There are 2 deadlines that must be noted (extended):

Submission of registration form: 13 March 2014, 6pm

Payment of S$15 per participant: 27 March 2014
